Questions about getting forms filled out
Sometimes, your employer needs a form signed by Dr. Kravitz. Or your school needs an excuse letter. Although we prefer you have these forms with you during your appointment, we understand that is not always possible. You may submit your form or request to our office via e-mail, fax, or by bringing it in personally. Please allow at least 5-7 days for completion - although often we can get it to you quicker. Please note* There may be a charge for this service.
How do I get refills?
Routine refills require 48 hours notification, so don't wait until you've run out of medicine before contacting us! You may call us yourself or have your pharmacy request the refill for you. As always, if it is an emergency or a new problem arises, please call our office immediately.
What billing or insurance information will I receive?
We do our absolute best to work with your insurance plan(s) so that you can concentrate on getting better! The only bill you will receive from our office is after your insurance has paid their portion(s) and the remaining balance is your responsibility. Please feel free to contact Susan or Jackie in our billing department if you have any questions regarding Dr. Kravitz' statement.
How long will it take to get things settled with the insurance company?
Your Explanation of Benefits (EOB) will be sent to you directly from your insurance company. Most insurance companies complete their claims within 30-60 days from the date of service. If you have not received your EOB or have questions regarding how they paid your claim, please call us.
Why does my EOB state "Surgery" if I only saw the doctor in his office?
Sometimes, insurance companies classify procedures - even those performed in the office - as "Surgery". Don't be alarmed... We know it's confusing and will do our best to explain it to you if need be.
How can I pay my bill?
We make every effort to collect your applicable co-pays and deductibles at the time of service; however, there may be times when you need to pay your bill when you're not in our office. Please click on the link below for fast, secure, on-line payment or mail your check to our office. Remember to reference your chart number on your payment, please.
Why does my statement say I owe more money than I thought?
Annually, we verify your insurance benefits. Unfortunately, the representatives or website are not always correct and as insurance companies are famous for saying, "This does not guarantee payment.". If you receive a statement with an amount owed that is different than you expected, please feel free to call our office and Susan or Jackie will be happy to explain it to you.
What if I don't have insurance?
Yuma Foot & Ankle Surgeons offers a discount for those patients without insurance coverage. Please inquire when making your appointment or when you check-in about this cash discount. Susan or Jackie will help you fill out a payment arrangement agreement should it be needed. Our #1 goal is to get you into the Doctor so we can help you back on your feet!
What is a NO SHOW fee?
If any appointment is not cancelled or rescheduled at least 24 hours prior, a $25 NO SHOW FEE may be assessed and billed to the patient. This is explained further in our Office Policies. Please contact either Susan or Jackie if you have any further questions regarding this charge.
What happens if I do NOT complete pre-registration?
All appointments are cancelled and may be rescheduled once the required pre-registration forms have been fully completed and turned in. The required paperwork can be completed in one of the following ways:
1. Download forms here and bring completed forms to our office.
2. Use the online portal to complete your paperwork electronically.
3. Paperwork may be picked up from our office during normal business hours.
